LeaderStat is partnering with TOP RATED senior living community in Willoughby, OH who is seeking a stellar MDS Coordinator for their skilled nursing.

This operator is committed to delivering exceptional care to residents and they pride themselves on maintaining a supportive team environment where staff are respected, valued, and given the resources to succeed.

Position Summary:

The MDS Coordinator is responsible for the timely and accurate completion of the Minimum Data Set (MDS) assessments and care plans in accordance with federal and state regulations and company policies. The ideal candidate is detail-oriented, highly organized, and knowledgeable about nursing home regulations and reimbursement systems.

Key Responsibilities:

  • Coordinate and oversee the completion of the RAI (Resident Assessment Instrument) process.
  • Ensure MDS assessments and care plans are accurate, timely, and in compliance with federal and state guidelines.
  • Collaborate with interdisciplinary team (IDT) members to gather assessment data and support resident-centered care planning.
  • Monitor documentation for accuracy and consistency with MDS data.
  • Submit MDS data to appropriate governmental agencies in accordance with required timelines.
  • Provide education to staff regarding MDS processes and documentation standards.
  • Participate in quality improvement initiatives and audits.
  • Assist in ensuring compliance with Medicare/Medicaid requirements.

Qualifications:

  • Licensed Registered Nurse (RN) in Ohio - active and in good standing
  • 2+ years of experience as an MDS Coordinator
  • Knowledge of Medicare PPS, Medicaid, and reimbursement systems.
  • Strong organizational, analytical, and communication skills.
  • Proficiency in electronic medical record (EMR) systems such as PointClickCare, MatrixCare, or similar.
  • RAC-CT certification preferred
